The Wellbeing Service provides patient-centred care and support: care that treats people as equal partners, focuses on personal outcomes, supports the management of their own health and wellbeing, and recognises the importance of prevention and anticipatory care and support.
Through self -management, this service helps adults who are trying to manage social, financial, health and other matters which weight heavily upon them. Please ask your GP or nurse if you wish to be referred to this service.
